RN Case Manager

Interim HealthCare- Maine is seeking a dedicated and compassionate RN Case Manager to join our team in Waterboro, ME. This role involves coordinating patient care, providing clinical expertise, and ensuring quality health outcomes for our clients. The ideal candidate will be proactive in managing case loads, communicating effectively with patients and healthcare providers, and advocating for patient needs.

Responsibilities

Assess, plan, implement, and evaluate patient care in collaboration with healthcare teams. Coordinate and manage patient case load to ensure timely and appropriate care delivery. Communicate effectively with patients, families, and healthcare providers to facilitate care planning and delivery.

Maintain accurate and detailed patient records in compliance with company and regulatory standards. Provide patient

education

and support to enhance health outcomes. Advocate for patients’ needs and coordinate community resources as needed.

Requirements

Current and valid RN license in the state of Maine. Previous case management or nursing

experience

preferred. Strong clinical assessment and critical thinking

skills

. Excellent communication and interpersonal abilities. Ability to work independently and manage multiple priorities.

Reliable transportation to visit patients in Waterboro, ME area.
