Opportunity Information Sutter West Bay Medical Group (SWBMG) is seeking a full-time Nurse Practitioner to join our Cardiology team at 1580 Valencia Street in San Francisco, California. This role supports two cardiologists in a growing outpatient practice and is essential to improving patient access and care continuity.
About
the Opportunity The NP will manage follow-up visits for established patients, allowing cardiologists to focus on new referrals and complex cases. The NP will also support transitional care efforts and provide timely post-discharge visits for patients with heart failure. A key part of this role includes educating patients on cardiovascular conditions, treatment plans, and lifestyle modifications to support long-term health.

Qualifications
Education
