First dates needing coverage [Required]: July 16 8a - July 19 8a = 72 hour blockNext available dates if candidate is truly stellar they can add on: July 19 8a - July 24 8a = 192 hour block totalSchedule: 24 hour call and rounding. Flexibility on scheduling is preferred.EMR: EPIC90 day credentialing timeframeMust be NCC certified.Admits for Level II NICU rounding discharge high risk deliveries intubation UVC lines.Bill Rates Needed for must be included in the ratesOrientation: 175 rate cap24 Hour On Call: 1872 rate cap to include 10 gratis hoursHoliday 24 Hour On Call: 2808 to include 10 gratis hoursCall Back: 182 per hour after 10 gratis hoursHoliday Call Back: 273 rate cap after 10 gratis hoursConversion fee: 15k rate capTier 1: 24 hour call: 1700 and below Callback: 170 and below Tier 2: 24 hour call: 1701-1872 Callback: 171-182 Travel - 750/round trip cap to include all taxes and feesLodging - 179/night cap for hotels to include all taxes and fees.
Preferred Lodging: Candlewood Suites Boise – Towne Square an IHG Hotel.Rental Car - To be reimbursed by facility not to be used with personal car/mileage reimbursement. 50/day cap including all taxes and fees regardless of car class. SUV/upgrades over cap will need to be preapproved will only be approved if needed for inclement weather.Mileage - IRS Standard Rate to be reimbursed by facility only to be used with personal vehicle not to be used with rental car.*Please Note: Trinity Health does not reimburse for any application or credentialing fees.
