Registered Nurse - Case Manager

Base pay range

$55.00/hr - $60.00/hr

Duties

• Assess patients, coordinate care, and plan discharges.
• Organize and participate in family meetings.
• Contribute to the development of treatment plans.
• Arrange DME, SNF, and ambulance transport independently.
• Utilize computerized documentation systems.
• Work with HMOs, IPAs, and understand adoption/foster care, drug/alcohol abuse treatment, mental health crises, and protective services.

Experience

• 2 years of acute medical Care Manager experience in Care Coordination
• Travel experience required
• Preferred experience in Trauma Level I and II
