Registered Nurse Case Manager

CarePartners is seeking a compassionate Registered Nurse Case Manager to join our Home Health team. In this role, you will coordinate and deliver high-quality, patient-centered care to members in their homes across Brevard County. Responsibilities include conducting comprehensive assessments, developing and implementing care plans, collaborating with providers, and educating patients and families to support optimal health outcomes.

You’ll work closely with an interdisciplinary team, ensuring continuity of care and efficient service delivery. The ideal candidate has strong clinical skills, excellent communication, and a passion for community health improvement.
