Position Summary

The Licensed Integrated Mental Health Therapist (BHN) works within a multidisciplinary primary care clinic, utilizing the Collaborative Care model. The therapist provides assessment, treatment planning and psychosocial therapy to a diverse patient population, documenting care and referencing community resources.

Responsibilities

• Assess patient symptoms and create customized treatment plans based on individual needs.
• Screen patients for common behavioral health disorders using PHQ‑9, GAD‑7, Columbia Suicide Risk Assessment, or other appropriate tools.
• Monitor patients for changes in clinical symptoms and treatment side effects, aiming for symptom reduction.
• Support psychotropic medication management prescribed by PCPs, focusing on adherence, side effects, and effectiveness.
• Track patient follow‑up and clinical outcomes using a registry, re‑engaging patients as needed.
• Facilitate care plan changes for patients not improving, including medication or psychosocial adjustments or appropriate referrals.
• Manage behavioral health crises arising in the primary care setting.
• Provide flexible behavioral health services to patients on and off the caseload, through scheduled appointments and immediate interventions via warm handoffs during medical visits.

Qualifications

• Master’s degree from an accredited institution in Social Work, Counseling, Marriage & Family Therapy, or related field.
• A licensure as a Mental Health Professional (LMFT, LMHC, or LICSW) defined within HB 1724.
• Minimum two (2) years of experience in a behavioral health setting.

Benefits

Competitive salary range: $38.38–$55.23 per hour, adjusted for experience and qualifications. Benefits include medical, dental, retirement plans, paid time off, and the Employee Assistance Program. Sign‑on bonus up to $5,000 and relocation bonus up to $10,000 may be available.
