Job DescriptionJob DescriptionA Spanish version of this position is available upon request La version de esta posicion esta disponible en Español si es requeridaBHSN, one of the fastest-growing organizations providing whole-person care in the region, is in search of passionate individuals to join our rapidly expanding team!Your role at BHSN:As a Supported Housing Case Worker at BHSN, you will assist eligible candidates, maintain independent living in transitional housing for homeless individuals and assist with empowering tenants to work toward self-sufficiency. All shifts are “stay awake” shifts. Work Schedule: Part-Time 20-29 hours a week, work hours will be 10am to 4pm within work week of Monday-Friday (days of week to be determined)What your day might look like:Provide clerical duties including but not limited to data entry, filing, answering phones, as neededAssist with admitting and discharging program participantsAssist with unit turnover which may include but not limited to removing garbage, cleaning, stocking until with furniture and household necessities.Assist with ensuring that tenants have NARCAN available to them on-siteMaintain open communication with Clinton County Department of Social Services and their contracted agencies, regarding openings and tenant needs.Work together with CCDSS to empower tenants to work toward self-sufficiency and secure permanent housingComplete assessments to identify residents’ needs and connect/link to services accordinglyAssist participants to develop individualized Housing Support PlansRoutinely meet with residents in their home environment Complete inspections of units to ensure dwelling is safe and meets Housing Quality Standards (HQS)Attend treatment team meetings as scheduled and collaborate with team members including internal and external support systems within the community to identify and connect to needed resources/supportsRespond immediately to crisis situations and offer intervention support in conjunction with BHSN Crisis and/or treatment team providers to prevent unnecessary ED visits Provides ongoing support and referrals as neededAct as a liaison between the tenant and the MHAB Property Manager to resolve housing issues quickly Complies with BHSN and regulatory policies and procedures Attends all training as requiredPerforms job duties as assigned within the scope of this descriptionThis position requires cross training at other BHSN residential programs for support services and staff coverage as neededParticipates in staff meetings as scheduledYour skills and qualifications:Associates degree in relevant Human Services OR High School diploma and 3 years-experience in human services required Excellent interpersonal, engagement and organizational skills Ability to work independently and collaboratively Strong written and verbal communication skills Valid NYS driver’s license with driving record acceptable to BHSN insurance carrier required Reliable transportation to travel locally Good computer/data entry skills Ability to lift up to 40lbs, bend and climb stairs requiredNARCAN and CPR certification required within 90 days of employmentWhat’s in it for you?Generous benefits, including personalized health coverage, paid time off, and holiday payWorking within our community, making a real impact, working alongside passionate colleaguesEducation reimbursement for specific program opportunitiesAccessible leadership team, coaching for your growth, and ample training opportunitiesWith BHSN University, we provide our team members with continuous learning, mentorship, and internal education opportunities to encourage our team to continue discovering and innovatingAs a rapidly growing organization, there are endless opportunities to grow within the organizationCommunity discounts, loan forgiveness & moreBHSN is an Equal Opportunity Employer, and supports Diversity, Equity, and Inclusion in its hiring and employment practices so that every team member can feel like they belong and be their authentic self to thrive in their personal and professional lives.
